  • If you already have an e-Services account for another health plan managed by Carelon Behavioral Health, you will need to create another e-Services account, with a new username, specific to the Fallon Total Care membership. (We are not able to link the accounts at this time, as the network is contracted by Fallon Total Care).
  • Using e-Services you will be able to submit requests for authorization for Psychological Testing, Methadone Maintenance, Day Treatment, and Detox.
  • The Outpatient Review form is not available here because authorization is not required for Fallon Total Care members for Outpatient services provided by in network providers.
  • Please note that Carelon Behavioral Health does not pay claims on behalf of Fallon Total Care members.
